Led by family physician Dr. Douglas Olsen, we provide complete primary care for teenagers and adults. From annual wellness exams and preventive screenings to chronic disease management and everyday health concerns, we are here to support your long-term wellness. We accept most insurance plans and are currently welcoming new patients.
In addition to family medicine, we’ve offered aesthetic services for more than 15 years, starting with laser hair removal and gradually expanding our offerings to include advanced skin treatments and rejuvenation procedures. Through our in-house MedSpa, O&O Laser, we provide non-invasive solutions for wrinkles, dark spots, scars, cellulite, and more, all with the same level of care and professionalism you expect from a medical office.

Meet Our Physicians
Douglas Olsen, MD
Dr. Olsen was raised in Hilo and graduated from Waiakea High School. He earned his BS in Chemistry at UH Mānoa, followed by his MD from the UH John A. Burns School of Medicine in 1996. After completing his Family Practice residency in Seattle, he returned to Hilo in 1999, first practicing with Straub before opening this private practice in 2003.
Kara Okahara, MD
Born and raised in Hilo, Dr. Okahara is also a Waiakea High School graduate. She earned her BA in Biology from UH Mānoa and completed her medical degree at the UH John A. Burns School of Medicine, followed by a Family Practice residency. Returning home in 1999, she has balanced medical practice with raising a family and now focuses on the aesthetic and administrative side of the clinic.

With over 30 years of experience in the medical field, Sharon is often the first friendly face patients see when they walk through our doors. She began her career as a medical assistant before moving into front-office coordination, giving her a unique understanding of both clinical and administrative care. Patients appreciate her warm personality, clear communication, and ability to make every visit feel seamless.
Kim — Medical Assistant
& Aesthetic Technician
Kim brings more than 30 years of hands-on experience to primary care and 15 years to aesthetic services. Known for her reliability and attention to detail, she assists in clinical care and performs aesthetic treatments with a gentle, reassuring touch. Patients often remark on her compassion, work ethic, and the calm confidence she brings to every appointment.
